Please print Pre-Registration form and complete all applicable sections.  You can scan and attach it to the contact form, e-mail it to Kate@Ketchum-llc.com, or mail it with a check or money order.  Once your deposit is received, you will be placed on the class roster.  Full payment is due prior to class start date.
